The certificate of destruction is a formal document that evidences the secure destruction of your paper records and hard drives.  This document is important to keep on file to be compliant with Federal and State Privacy Laws. The certificate of destruction should include the following information;

  • The client’s name and address
  • Date of the Destruction
  • Location the destruction was performed
  • Shredding company’s representative that performed the destruction with signature
  • Client’s representative that was present with signature
  • Description of how the destruction was performed
  • Description of what was destroyed

It is important to note that you should have some type of supporting documents, in addition to the certificate of destruction, that give specific detail of what paper records were destroyed such as “Payroll Records 2009”.  It will not be necessary for hard drives since the certificate of destruction will list the hard drives by serial number.
